Program Overview

North Fulton Hospital instituted the Senior Care Project to help accommodate the needs of the rapidly growing senior community.

Through patient-centered care and a multidisciplinary team approach, the Senior Care Project seeks to:

  • Help senior patients maintain independence and minimize functional decline
  • Increase patient/family education

This comprehensive program incorporates two components to serve the seniors in our community:

  1. Care Transitions Task Force – regular meetings with all senior living communities, skilled nursing facilities and other community health care providers to increase awareness and education on patient transitions and quality of care.
  2. Physician Provider Network – network of community providers who are willing to see Medicare patients requiring an office visit within 48 hours; patients are connected through a call center at North Fulton Hospital.

To find a physician in the Physician Provider Network, please call 770-751-2600.
