At North Fulton Hospital’s inpatient rehabilitation unit, we understand that beginning a comprehensive rehabilitation program can be a difficult and sometimes uncertain time. That’s why we have assembled a team of thoughtful, caring professionals who will be there every step of the way. Our experienced staff has been providing rehabilitation services for brain injury, stroke, multiple trauma, orthopedic injuries, spinal cord injuries and general medical patients since 1984. In keeping with our commitment to delivering quality care, we were certified as a Primary Stroke Center in 2005 by the Joint Commission on Accreditation of Healthcare Organizations.
Should you require additional inpatient therapy and nursing services following your hospital stay for an acute condition (neurological, complicated medical, multiple trauma, and others), our on-site multi-disciplinary rehabilitation unit offers a focused team approach for your continued recovery towards functional independence.

Inpatient Admission
In order to be accepted for admission, a patient must meet the following criteria:
• Require a coordinated interdisciplinary plan of care that includes two or more therapies;
• Be motivated and able to participate in a minimum of three hours of therapy per day, five or more days per week;
• Have the potential to improve his or her functional abilities with the goal to return home or to a community-based environment;;
• Require 24-hour availability of a physician specializing in Physical and Rehabilitative Medicine;
• Require 24-hour availability of a registered nurse with specialized training in rehabilitation;
• Have the medical necessity to be in an acute rehabilitation setting;
• Be 15 years of age or older.
The Patient Referral Process
• Once a referral has been made by a physician, an admissions liaison will conduct a pre-admission assessment;
• Upon completion of the assessment, a rehabilitation physician will determine the appropriateness of the patient for admission;
• The referring facility will be notified of the patient’s acceptance or rejection to the program;
• The referring facility will arrange for transportation to NFH.
Rehabilitation Team
The rehabilitation team at North Fulton Hospital is normally made up of the following healthcare professionals:
• Rehabilitation Physician
• Physical Therapist
• Occupational Therapist
• Speech/Language Pathologist
• Rehabilitation Nurse
• Case Manager
• Dietitian
The rehabilitation team meets weekly to discuss the patient’s progress, barriers to treatment and discharge plans. Individualized goals and treatment plans are established and reflect patient/family participation.
Patient/Family Participation
It is important that a family member and/or caregiver accompany the patient upon admission, ideally for at least one-half day, to register and give information pertinent to the patient’s individual needs. Also essential to the success of the rehabilitation stay is family support and participation. During the inpatient rehabilitation process, it is important that patients have several comfortable/loose fitting outfits and non-slip/sturdy shoes (athletic shoes work well).
Family members and/or caregivers will be asked to attend scheduled family training with therapists and nurses prior to discharge. This provides the patient and family an opportunity to meet appropriate team members, to discuss the patient’s progress and clear up any concerns that may arise. A patient’s transition home should be a safe one for both the patient and his or her family members.
Discharge Plans
The discharge planning process begins immediately upon admission and involves the patient, family members, and rehabilitation team. Individual progress and needs are addressed at the team meetings and family sessions. Family conferences are scheduled as needed. The case manager and therapists work with the patient to obtain the necessary home medical equipment and services upon discharge.
