Program Overview
North Fulton Hospital instituted the Senior Care Project to help accommodate the needs of the rapidly growing senior community.
Through patient-centered care and a multidisciplinary team approach, the Senior Care Project seeks to:
This comprehensive program incorporates two components to serve the seniors in our community:

Care Transitions Task Force – regular meetings with all senior living communities, skilled nursing facilities and other community health care providers to increase awareness and education on patient transitions and quality of care.

Physician Provider Network – network of community providers who are willing to see Medicare patients requiring an office visit within 48 hours; patients are connected through a call center at North Fulton Hospital.

Senior residents of Fulton County may be eligible for various transportation options. Individuals must meet certain criteria in order to meet the eligibility requirements for these programs. Because these are not North Fulton Hospital programs, we cannot guarantee their quality or reliability.

Who is eligible: Medicare recipients, age 60+
Destinations: Medical appointments
Frequency: Subscription service available for specific times, i.e. chemotherapy, 3x/week for dialysis or 2x/month for other medical appointments
Lead Time: 2 - 4 weeks in advance

- Jewish Escorted Transportation Services (JETS) For Seniors in North Fulton
Provided by: Jewish Family & Career Services—Escorted Transportation Services
Who is eligible: Age 60+
Service Area: Zip codes 30075 and some of 30076, within a 12 mile radius of the JETS Facility in Dunwoody
Destinations: Medical appointments. Other types of requests will be considered based on availability
Wheelchair Accessible: Yes
Cost: Within 12 miles: $11.00—$12.00 each way; with wheelchair and escort, $20.00 each way. Beyond 12 miles, an additional fee applies

Who is eligible: Persons with disabilities who are, because of their disability, unable to board, ride or disembark from an accessible vehicle on MARTA’s regular bus or rail service
Service Area: Within Fulton and Dekalb counties along a 3/4 mile corridor located on each side of all fixed bus routes and within a 3/4 of a mile radius of each station
Wheelchair Accessible: Special lift-equipped vans are provided on a curb-to-curb, shared ride basis
Cost: $3.50 each way. Personal attendant travels free
